Falls are a major cause of injury for older people, accounting for 56% of hospital admissions for people who are 75 years of age and older. Falls are the cause of over one-third of all deaths related to injury among people aged 55 years and over. Almost half of all falls occur at home. Significant research confirms the cost effectiveness of implementing prevention strategies in the falls risk area for people over 65 years of age and living in their own homes. As referred to above, Rehability works with many public and private hospitals, making life easier for both hospitals and patients by providing a key role in the ‘continuity of care pathway’ from hospital (or other health setting) back to the patient’s home. Research has demonstrated that the provision of Physiotherapy and/or Occupational Therapy in the home post-discharge from hospital is a highly effective and appropriate health management option providing it is assessed appropriately for each individual patient and their specific needs.
Funded Programs
A wide range of care coordinated funded programs purchase services from Rehability, including the Post Acute Care (PAC) program, the Hospital In The Home (HITH) program, Commonwealth Rehabilitation Service (CRS), and the Extended Aged Care in the Home (EACH) program.
In addition to these programs, Rehability also provides services to private hospitals and to clients funded by the Department of Veteran's Affairs (DVA), Victorian WorkCover Authority (VWA) and the Transport Accident Commission (TAC).
Rehability has been providing services to clients in their own home through the provision of community-based physiotherapy, occupational therapy and rehabilitation since 1997. Today, Rehability's services have extended to include:
