Privacy Statement

Rehability is subject to the Privacy Amendment (Private Sector) Act 2000 and the Health Records Act 2001.

  1. Information Collection

    Rehability collects personal information from agencies that have referred you to us to provide rehabilitation services for you. The personal information we collect may include your personal details, information about your health and services you may use or require and information about your living arrangements and lifestyle.
  2. Information use and disclosure

    Rehability will use your personal information to allocate the most appropriate rehabilitation provider to meet your needs. We will also use it to invoice the agency purchasing services for you.

    Rehability will not provide your personal information to any other party or use this information for any purpose other than that for which it was originally intended, without your consent, unless we have a legal obligation to do so or in an emergency situation where you may be endangered and not able to provide consent.
  3. Information security and retention

    Rehability uses a variety of security measures to safeguard your personal information and keep it secure from misuse, loss or unauthorised use. Access to our offices, files and electronic databases is restricted to staff that require this information to effectively perform their roles.

    Rehability will retain personal information for as long as it is needed for the purpose it was collected or any other purpose authorised by the Act or any other law.
  4. Information Quality and Accuracy

    Rehability will endeavour to ensure the personal information we hold is accurate, complete and up-to-date. However, the accuracy of the information we hold largely depends on the information you provide. Please inform your treating Rehabilitation provider, referring agency or Rehability if you are aware of any errors in your personal information.
  5. Information Access and Correction

    You have the right to access your personal information subject to some exceptions allowed by law, and to request a change to your information if you consider it is inaccurate. You may contact Rehability if you would like to access your personal information.

    In some circumstances, where the information is in the past and has been archived, Rehability may charge a fee for searching for and providing access to your information.
  6. Making information available to other health providers

    If clients request that information held about them is made available to another health provider, Rehability shall provide a copy or written summary to the other health provider on payment of a fee.
