  • Falls Prevention - Occupational Therapy in your own home

  • Falls are a major cause of injury for older people, accounting for 56% of hospital admissions for people who are 75 years of age and older. Falls are the cause of over one-third of all deaths related to injury among people aged 55 years and over. Almost half of all falls occur at home. Significant research confirms the cost effectiveness of implementing prevention strategies in the falls risk area for people over 65 years of age and living in their own homes. A large number of publicly funded programs are implementing preventative programs for their 'at risk' populations.

  • Equipment and Aids Prescription

  • Rehability's physiotherapists and occupational therapists are highly trained in assessing and identifying the most suitable form of aid or equipment to allow clients to maximise their safety, comfort and independence at home and in the community.

  • Occupational Therapy for safety in the home

  • A home safety assessment is a comprehensive assessment of the home environment and the ability of people who live there to manage safely and independently. Home safety assessments have shown to significantly reduce the risk of falls and injury in the home. They may also prevent the need for hospitalisation.

  • Supporting the effective discharge from hospital to your own home.

  • Rehability works with many public and private hospitals, making life easier for both hospitals and patients by providing a key role in the 'continuity of care pathway' from hospital (or health setting) back to the patient's home.

Research has demonstrated that the provision of occupational therapy in the home post-discharge from hospital is a highly effective and appropriate health management option providing it is assessed appropriately for each individual patient and their specific needs.
